During your first visit, we will need to obtain important background information including your medical history and current medications. We will also need to know the name of your Primary Care Physician (PCP) and the date of your last medical visit. Our staff will be happy to help you through the initial paperwork if needed. Once you are in our system, follow-up visits are very streamlined. We use an Electronic Health Record (EHR) system which allows us to easily update and maintain your health information. This system also allows you to securely access certain information on-line via the Patient Portal . This feature can be setup for you at your initial or follow up visits.
THINGS YOU WILL NEED TO BRING TO YOUR FIRST OFFICE VISIT INCLUDE:
- Insurance Cards
- Photo I.D.
- Current medication list
- Name of your Primary Care Physician and date last seen
